The HealthMedia® Move® and HealthMedia® Balance® workshops are administered by HealthMedia, Inc. (HealthMedia®), a Johnson & Johnson affiliate. If you decide to participate in the workshop, you will receive a tailored behavior change intervention guide and follow-up communications. In order to provide you with an accurate and useful tailored personal guide, HealthMedia® asks for the minimally necessary information which includes your name, address, and relevant health history along with information about lifestyle behaviors such as exercise, physical measurements such as height, weight, and blood pressure, and eating habits topics. Your answers form the basis of the personal guide that is unique to you. In conjunction with the personal guide you may also receive follow-up newsletters and emails. Email communications will be sent to the email address you provide. These communications may contain information about your condition or disease state. It is the policy of McNeil Nutritionals, LLC and HealthMedia® to not voluntarily disclose your personal health information to your employer, insurance company, or any other third party without your specific authorization unless required by law.
McNeil Nutritionals, LLC and HealthMedia® will create aggregate data results for dissemination to other organizations and for research purposes. THIS AGGREGATE DATA WILL NOT CONTAIN ANY INFORMATION WHICH DISCLOSES YOUR IDENTITY. Nutrihand, Inc.
The MealManager program meal planning tool is administered by Nutrihand, Inc., a third-party affiliate. If you decide to participate in the program, you can choose to receive 5 days of daily tailored meal plans for your specific caloric level of interest. In order to provide you with accurate and useful tailored meal plans, Nutrihand, Inc., asks for the minimally necessary information which includes your name, gender, height, weight, eating habits, and weight management/weight loss goals. Your answers form the basis of the caloric level that is deemed appropriate for you based on calculations for a healthy adult. The meal plans provided in this program are not designed for specific medical conditions. We recommend that you speak with your healthcare provider if you have a specific medical condition, to address your individualized dietary needs. In conjunction with the program you may also receive follow-up newsletters and emails. Email communications will be sent to the email address you provide. These communications may contain information about your condition or disease state. It is the policy of McNeil Nutritionals, LLC and Nutrihand, Inc., to not voluntarily disclose your personal health information to any other third party without your specific authorization unless required by law.
